Grace Community Church Facility Use Request Form

Please schedule your event at least four weeks ahead of time to ensure the date you want is available. Do not announce your event until the availability of the date has been confirmed. If you are requesting use of the facility for a wedding please contact the church office.

Important! When you are done, please scroll down and click on "Submit Form" to send your form to the office. If you don't see the message "Your form has been sent", then look for the red error message to see what information still needs to be entered correctly.  Please review ALL the fields below and fill them out appropriately.
